APPLICATION FORM

I declare that all the information contained in this application form and any additional sheets attached are true and accurate and that I have not withheld or misrepresented any material facts. I understand that if I am employed, any misrepresentation or withholding of material facts required in this application form shall be sufficient cause for Hanbaobao Pte. Ltd. (Licensee of McDonald’s) ("the Company") to terminate the employment contract with immediate effect. I hereby agree and consent for the Company to obtain references or opinions about me from my character referees and employers, both past and present.

I hereby authorise, agree and consent for:

• the Company and its affiliates (collectively "McDonald’s") to collect, use, disclose and/or process my personal data that I now provide to McDonald’s in this form; and
• McDonald’s to disclose my personal data to its agents and third party service providers (collectively "Representatives"), whether such Representatives are located within or outside Singapore, in order for McDonald’s Representatives to perform certain functions;

in connection with my job application to work in McDonald’s.

I hereby represent and warrant that where I provide the personal data of other individuals to the Company, I have obtained the consent of such individuals to disclose their personal data to the Company for the purposes as indicated in this application form.
